2007 Tournament Team
Agreement will be required for US Club
teams only, as well as all items listed on the
Agreement. Read this document carefully.
The additional items required by this document
include, but are not limited to, a US Club
certificate of liability insurance and an IYSA
disclosure statement for all adults associated with
the team. |

To speed up your on-site registration, you may mail
copies of the following documents: State Approved
Roster with any add/drop forms, IYSA Medical
Waivers, Travel Permits (if applicable), IYSA Sanctioned Team Roster,
and the documents for US Club teams related to the
2007 Tournament Team Agreement. These documents
